"Originally prepared by the Committee on health and medical relief for the United States Railroad administration. It was submitted to the surgeon-general of the United States Public health service and such changes were made as were necessary to bring it into accord with the interstate quarantine regulations. Some amendments were also made by the Conference of state and provincial health authorities and it is now published as approved by the Conference of state and provincial health authorities for the information of health officials." - p. 13.
